In part 1 of the form, under "Current USCIS status" should I put F-1 ? And then in the next box for expiration date, should I put "D/S" which is on my I-94, or the visa expiration date on my visa, which has already expired?

You should write Visa K1 holder, and expiration date is in I-94 (three months since you arrive) even if it is expired.

Yes, put F-1 or F-1 OPT if you have the OPT
