Hello All,

I'm ready to submit my I130 and I485 for my adjustment of status, what forms are required to fill other than 130,485 combo? Do i also need to send my medical examination report with the forms or do i have to take that with me when I'm called for the interview?

I believe i will have to fill out I693 form for that? please help and thank you in advance.

You will also need three G-325As, one for your wife two for you.

Yes, you will also need to send in Form I693 to complete the filing, but it is a USCIS approved doctor who completes the form, not you.

You can also file forms I-131 and I-765 with the rest of the forms, they are not required but worth they are worth it.

I would suggest starting there and then coming back with any questions...

There are several more things you need to do besides filling out an i-130 and an I-485 and those things can vary from situation to situation...some people need to send more documents than others.

Your civil surgeon will be able to instruct you on the details of the I-693 - essentially he/she knows exactly what to include in that particular packet and will seal the envelope which you will include in your overall packet. Do not open it...but he/she will tell you that too.

Hello All,

There is one quite important - I-864. Go read the guides, you are nowhere close to being ready to send it out, as with I-864 you need to send a whole pack of supporting documents.

It is optional to do the I-765, which is the employment authorization. The employment authorization usually takes a little bit over 2 months. If you can wait to get the green card to work, then you dont need to file this form. However, it is of no additional cost when the I-485 is being filed. MY point is, it wouldnt hurt to file for it, its only one page.

Also, I advise you to READ and RE-READ the instructions on all the forms, because to me, it sounds like you havent read it. If you haven't you will miss A LOT of things and cause problems for yourself.

Thank you very much Chris for your prompt reply - do I need to apply for her employment card as well separately? or is that a part of the adjustment process where they send the card to the applicant after 90 days.

My apologies, I was typing from my cell and I missed off Form I-864 in my earlier reply. This a required form that is also needed for adjustment.

You don't *need* to apply for her employment card but if she wishes to work before obtaining her GC then go ahead and file the form-- it doesn't cost any extra when filed with the I-485 and it usually takes around 90 days to arrive in mail.
