I'm kind of lost here the aos packet states that another medical isn't required if the k1 beneficiary arrived with-in the last 12 months, but then it goes on to state form I693 is required.

How can anyone fill out this form accurately if during the medical exam the required vaccines were put into a sealed envelope and giving to the uscis?

any information would be greatly appreciated, thanks

If your first medical was complete then you are good to go, if more shots were needed then you should know what you need and get them when you are here.

The beneficiary is given the sealed vaccines records which shows what shots were given during their medical in their country. He/she may not need a medical. But if their shots were not complete they will need to see a CS to get the missing shots and have them transcribed into the I-693.

It's really up to you. I have heard of some people simply sending in the attached DS-3025 and not having a problem. (They peeked at the DS-3025) I myself wanted to be safe so we went to a Civil Surgeon since they are designated by the USCIS to look at the forms and complete any paperwork necessary for the packet. Hope this helps.

You don't personally fill the I-693 form. You shop around for a civil surgeon covering your jurisdiction & willing to transcribe the records from you DS 3025 Vaccination sheet into the I-693 form. This form is then sealed in an envelope and given to you to include in your AOS package. You are to get a copy of the DS3025 Vaccination sheet after medicals.

If your medical and vaccinations have been done in your country you should be fine as USCIS will have it then (hopefully it is not lost)

I didn't send it. I just sent a copy of Dr. Isis Cazali (Guatemalan Embassy Dr) vaccine list, where it stated that I was to date with my shots (DS-3205). I also sent them a letter explaining why I did not need the I-693.
