Follow the guide.

No you don't take it in person anywhere. Expedite for 131 AP permit is generally given for emergency travel such as death in the family.

And I-693 medical form...

But all this is very dependent upon your particular situation...

On what basis are you applying for AOS ? What did you tell the border patrol about why you were coming over and how did that change?

Oh and you will not be taking it in person anywhere. Follow the forms directions on wher to mail it and get a next day deliver with guarantee.

You will need three G-325As, one for your wife to accompany the I-130 and two for you to accompany the I-130 and I-485.

You will also need Form I-693 which is completed for by an USCIS approved civil surgeon.

Regarding expediting Form I-131, this is taken from the instructions to the Form:

"To request expedited processing of applications for Re-entry Permit or Refugee Travel Document, write the word EXPEDITE in the top right corner of the application in black ink. We recommend providing e-mail addresses and a fax number with any expedite request for the Re-entry Permit or Refugee Travel Document"

http://www.uscis.gov/files/form/i-131instr.pdf

I highly suggest that you carefully read ALL of the instructions to each form before completing and filing them, all the information you need is there.

when does this form have to be sent in?

The I-693 gets sent in with the the entire package.

You will go to your USCIS approved Doctor and it is he/she that will complete Form I-693 and they will then seal it in an envelope with a piece of tape covering their signature.

You cannot file Form I-485 (Adjustment of Status) without a completed medical and I-693 since this Form is required to have a complete AOS package.

You will file all of the Forms listed above, together in one package and send them off to the Chicago Lockbox. Filings Forms I-130/I-485 is called "concurrent filing"
