Form I-751 part 3, question 9

Hi there!!!

I'm very close to send my I-751 application. However, I have reviewed the form I-751 and I just wanted to double check with you guys the answer for question 9, part 3.

"Have you resided at any other address since you became a permanent resident?If yes, attach a list of all addresses and dates".

When I received my conditional GC, we have lived at address X. After about 6 months, we have moved at adddress Y and I followed the USCIS change of address procedure. My question is: do I include both addresses X and Y in my answer to question 9 or do I include only my recent address?I'm confused...

Include your current address on form I-751 and list your previous address on an additional page as instructed in the question.
