I found a doctor from the USCIS Designated Civil Surgeons List. My fiance only needs the last shot for the mumps. They only want to give a physical. I'm confused-- does he have to have the whole examination again?

He has two shots for the mumps in Thailand and they told him he needs one more on March 7th.

Currently.... they have issued Ice the incorrect GC and we have tried 4 times to fix it. First time they had us send it to the incorrect address. Second time they said we used an expired form, which was the form they gave us. Third time was "oh sorry we lost the last page, can you send it again?." Fourth time is the gov is shut down. Will this ever be corrected in time for Ice to get the permanent GC? Stay tuned to find out. T_T

If his physical was less than 12 months ago he does not need the whole physical.

Many CS's want to complete the whole I-693 but it's not required. If his physical is still valid he only needs the vaccination portion completed. Any chance you can call another CS and see if they will do just a "supplement"?

Did he recieve a DS-3025?

Lots of folks don't get a DS-3025 because they choose not to have their vaccinations completed in their home country. You can have them completed in the US by your primary care giver (or a CS) and then have the CS sign off on the vaccination portion of the I-693.

He has two shots for the mumps in Thailand and they told him he needs one more on March 7th.

Take him to your local county health clinic. I had my follow up shots done at mine for 23 bucks. They issued me a current shot card as well. This isnt mandatory for the AOS if that is your concern.

"Incomplete vaccine history . Applicant may be eligible for waiver because vaccination not medically appropriate."

Then it indicates above that he needs one more shot on March 7th (his birthday!!!) in order to be complete.

@Ning- Does that mean I can just go anywhere to get the last shot? Or do I need to go to a C.S?

Plus the doctor's hand writing is horrible :lol:

I think we had a quick physical, blood test, TB test and read x-ray then done. I guess I could have been pickier but just wanted it done. All accepted no problem. All Dr handwriting horrible it is a requirement.

go to a local medical center near you, get his shot on march 7th. it will cost way less than a civil surgeon. then take a proof that he got his vaccine, attatch it to the DS-3025 and send it like that. mine is marked incomplete because many vaccines were not age appropriate or not required. didnt need any CS for that

edit: when you get the proof ask the doctor for a printed and hand signed one with the stamp. not hand written

It can be done anywhere shots are given. If you are upset with him take him to a Vet. If not & want to save money try the health clinic. My husband took myself & our daughter to ours. My shot cost 23 bucks instead of 200. My daughter needed a total of 9 to be enrolled in school. Those were done for free.

We took the form to the clinic. They stamped & signed it. We turned the form in at the AOS meeting. The officer didnt seem interested in it & didnt even look at it.

im guessing that is the 2nd MMR shot?? if it has a tick in the "insufficient time interval" and the rest of the form is filled in correctly then that 2nd shot isnt needed and you dont need to see a CS
