question abut form i-693 medical examination

Hello all, Im trying to complete my AOS and i have seen that if u had ur medical examination over seas within a year of filling the i-485 u dont need a new medical examination, but also it says i have to go see a civil surgeon anyway so i dont understand it very well.

Should i fill the i-693 my self with all the vaccination (i have all required ones) and add the recipe of the fees for medical examination i had in my country to prove it was on 02-01-2012 or i just have to make an appointment with a new surgeon here in the US and let them fill all the papers for me again?

No no no no no if you fill in the vaccination portion of the 693 by yourself and not a civil surgeon you will get an RFE for an improperly completed form! Do not do this!!!!

If you have had all vaccinations and your copy of vaccination sheet from your medical over seas is complete all you do is include a copy of this in the AOS package.

I also have a question about this:

I went to a civil surgeon today and told him I got the blood test and x-ray less than 6 months ago. He would not sign unless I had proof of the blood test and a hepatitis B shot. Thankfully I had that CD with my x-ray on it.

When I did my shots in canada, the hepatitis B shot was not required for my age group. Should I get it anyway?

Is there a way to get the record of my blood test from canada? (I did my physical in vancouver)

You need to find another civil surgeon then. None of that is required to fill out the vaccination portion.

Hello Inky and thx for ur answer!! let me clarify to see if i understood u.

I had all my vaccinations when i went to the medical overseas, also that doctor made a sheet where she took notes of each one of my vaccinations and the day i got them, and this is the sheet i gave to the Consul when i had the visa interview at my embassy (in other words, i dont have that sheet but it should b on my records somewhere in my country or here) So onlything i do, is just get a copy of my health book where all the vaccinations are, and atache it to the AOS pack?

Or as i dont have the sheet the doctor filled on my medicals overseas i have to go to another doctor in here?

Sorry to ask again, is just im not so sure, and this AOS looks kind of difficult for me>

Hep B is not required. You need at least one from MMR, one from Tdap/TD, varicella or a VH marked by it to indicate you had the chickenpox. Possible the flu shot if you had your medical between Oct. and Mar. If you have all those, even if the form is marked incomplete, may be eligible for blanket waiver, you can skip the CS and just send a copy of your DS-3025.

If you need shots, find a CS that does not insist on doing the medical over again. Blood work, TB test, Hep B? None of that is required since your medical exam was within the last year. Call around to find a CS that will do just the vaccinations on the form.

Since you do not have a copy of your DS-3025 from your medical, you will need to take your vaccination records to a civil surgeon and have them transcribe your vaccinations onto an I-693. Do not fill one out yourself. It has to be done by a civil surgeon. They fill it out, sign it, and seal it in an envelope. You will send that envelope in with your AOS paperwork. Call around until you find one that will agree to do JUST your vaccination transcriptions because you do not need another medical.

Since you do not have a copy of your DS-3025 from your medical, you will need to take your vaccination records to a civil surgeon and have them transcribe your vaccinations onto an I-693. Actually technically the DS-3025 isn't a requirement for AOS. The OP should be fine sending a letter stating their K1 medical was included in their A file handed over at POE. If shots are missing they'll get RFE'd.

Well in that case, if they know for sure they all the required vaccinations, then they can go ahead and make a statement about having all their vaccinations and another medical not being needed and that their medical records should already be in their file. I would try it over finding a CS, and not worry about it unless I got an RFE.

Agreed. Given they don't have their DS-3025 the CS might make them get the shots all over again so I'd try that first and see what happens but in the meantime try and get a hold of shot records and find a decently priced CS in their area in case they DO get an RFE
