For item C15 they asked for my husbands present employer name and address.

1) there is no place for the address do I add on an extra sheet? and what do I write on it?

2) it is an american NGO do I write the haiti address, the US address or both?

If space on the form is insufficient, write "see attached" on the form and write the information on the extra sheet. On the extra sheet, write the question number and question that is being asked prior to writing the appropriate information.

April 22, 2011: Mailed I-130 to Chicago
April 28, 2011: Received NOA1 via text/email, file routed to CSC (priority date April 25th)
April 29, 2011: Updated
May 3, 2011: Received NOA1 hardcopy in mail
July 26, 2011: Received NOA2 via text/email!!!
July 30, 2011: Received NOA2 hardcopy in mail
August 8, 2011: NVC received file
September 1, 2011: NVC case number assigned
September 2, 2011: AOS invoice received, OPTIN email for EP sent
September 7, 2011: Paid AOS bill (payment portal showed PAID on September 9, 2011)
September 8, 2011: OPTIN email accepted, GZO number assigned
September 10, 2011: Emailed AOS package
September 12, 2011: IV bill invoiced
September 13, 2011: Paid IV bill (payment portal showed PAID on September 14, 2011)
September 14, 2011: Emailed IV package
October 3, 2011: Emailed checklist response (checklist generated due to typo on Form DS-230)
October 6, 2011: Case complete at NVC
November 10, 2011: Interview - APPROVED!!!
December 7, 2011: POE - Sea-Tac Airport

September 17, 2013: Mailed I-751 to CSC

September 23, 2013: Received NOA1 in mail (receipt date September 19th)

October 16, 2013: Biometrics Appointment

January 28, 2014: Production of new Green Card ordered

February 3, 2014: New Green Card received; done with USCIS until fall of 2023*

December 18, 2023:  Filed I-90 to renew Green Card

December 21, 2023:  Production of new Green Card ordered - will be seeing USCIS again every 10 years for renewal

For 1), I always just write "PLEASE SEE ATTACHED FORM: "<NAME> EMPLOYMENT HISTORY"" and include a form with that exact name straight after the I-130 form.

Not sure on 2), sorry.

Apr 27 2011 - Interview attended at US Embassy, visa approved :D!
May 26 2011 - Arrived in the USA
Aug 02 2011 - Married!
Aug 24 2011 - Diagnosed with cancer
Aug 26 2011 - Applied for IR1/CR1 visa
Aug 31 2011 - NOA1!
Sep 03 2011 - Moved back to the UK
Feb 20 2012 - Wife flew back to the USA
Mar 03 2012 - NOA2!
Mar 23 2012 - NVC received file
Jul 15 2012 - NVC approved, sent to embassy

2012 - Approved, flew back to USA!

Sep 29 2014 - Applied for removal of conditions.

Apr 20 2015 - RFE1 on removal of conditions.

i have also read that you need to sign and date the extra sheet (with the same date you use to sign the forms). don't know if this is true because i didn't need an extra sheet for any of my forms. just something you may want to look into.

i would also assume you can probably also add an extra sheet explaining both addresses too, if you can't find out which one to use.

I signed the extra sheets.. The date was slightly different I think.
