maiden name means "name before marriage". So name at birth.

Don't leave anything blank. If she was never married, and never used another name, then put 'None'. See the example form: http://www.visajourney.com/examples/Form-I-129F.pdf

So your fiancee's mother has never been married and never changed her last name?

Okay you seem to be confusing matters with what others have told you.

From your original OP you ask about "other names used" if this section is the section that relates to your fiancee but she has never changed her name or USED another name then leave it blank.

You then mention her mother. If you're referring to the section where it asks for parents names then in her mothers box you would write her BIRTH (maiden) name in the box for last name (as the form states).
