As i applied for my AOS i send all of the documents except the original of the vaccination history that they gave me when i got my medical in the philippines.I just send them all the xerox copy, i dont know what what to do, i need to continue my vaccination here and i dont know what to do, as i enter the US i got the TD shot and then the DR. that give me the shot just give me the letter certifying that he gave me that shot that day, guys what do i need to do?

You have to go to a designated surgeon near your area to take or continue your vaccination and bring that letter of the record of your vaccination from Philippines. Click the link to find a designated surgeon by the USCIS near your area http://www.uscis.gov/graphics/exec/cs/index.asp

Aside from TD shot you should go to civil surgeon and the civil surgeon will transfer the data from st. lukes in I-693 supplemental form. The civil surgeon will sealed it.

What you did was you just photocopied it and you didnt go to civil surgeon.
