Hi,

So I'm almost ready to send in the paperwork for my husband's adjustment of status... but I'm very confused about the I-693...

Some people have posted that they just sent in the DS 3025 and didn't need to fill out the I-693 at all... However, at POE they took the whole packet that he got at his medical exam in Naples, which contained his vaccination record the DS 3025... Is it okay to assume that they already have it on file and I don't need to send it? If not, then I don't really know what to do...

We have a list of my husband's vaccinations he has had since birth from his family doctor (not a civil surgeon), would this suffice? Or should we take the list to a civil surgeon here and have them fill out the I-693?

Any help/advice would be very much appreciated!

4/4/2011- NOA1 email
4/11/2011- NOA1 hard-copy
8/3/2011- NOA2
10/12/2011- Interview and Visa Approval!

January 7 2012- wedding

07/22/14-Mailed I-751 to remove conditions

01/28/2015- removal of conditions approved

They do require the D3025 and when he had his medical they should have given him a copy of it. If not, it is my understanding that you will need the I693. Good luck.

No technically it's not required but including it seems a lot safer. Should have been given a copy at the K1 medical
