I am a US citizen applying for my fiance's visa. I was adopted as a child by my stepfather and received a new birth certificate; does anyone know if I need to write down my birth name on the 129? and then send supporting documentation (re: adoption paperwork,etc)?? Thanks!!

January 29, 2010: Met and maintained relationship as prayer partners and friends
January 2012: Realized there was more to this than friendship: Prayed to God for guidance
January 29, 2012. Final confirmation from God that this was of Him!
February 16, 2012: Told family and friends and Facebook world about our courtship
March 1-15 2012: Visit!!! and met family and friends.
March 9, 2012: Engaged!!!!
March 14th: Mailed Petition to USCIS
March 21, 2012: NOA1
