We are doing a spousal concurrent filing of i-130 and i-485 (so you'll obviously have to substitute your own situation into these) but this is what we used:

I just did a basic cover letter like this:

Dear Sir/Madam:

Enclosed is a concurrent filing of forms I-130 (Petition for Alien Relative) for my wife, I-485 (Application to Register Permanent Residence or Adjust Status), and all other supporting forms and documents necessary.

Contents include:

1. G-1145

2. I-130 (Check for $420)

3. G-325A (Myself and Wife)

4. Passport Style Photos (Myself and Wife)

5. Copy of my birth certificate

6. Copy of my wife’s birth certificate

7. Supporting evidence of marriage (photos, joint bank account, bank account beneficiary, marriage certificate)

8. Certified Marriage Certificate

9. I-693

10. I-485 (Check for $1070)

11. Copy of my wife’s birth certificate, wife’s biographical page in passport, B2 Visa with entry stamp into America, I-94 form (front and back)

12. G-325A (Wife)

13. Two Passport Style Photo of Beneficiary

14. I-864 of Petitioner (W-2 and Tax Return)

15. I-864 of Joint-Sponsor (W-2, 1099 and Tax Return)
