Hello Vjer's,

ok I am back and getting on with our AOS, we are in the middle of looking for a civil surgeon. While looking at some post I came across several that said we dont need to go see one if certain things on her DS-3025 from St Lukes were present. To be honest I am still confussed (old age ) lol. Any help would be appriciated. Under the result section , the box for vaccine history is incomplete, but so is the box saying applicant may be eligiable for a blanket waiver because vaccinations are not medically appropriate. The only thing i can see as being incomplete is Tdap vaccine for insuffient time, have no clue what that means. Anyway not sure if any of this makes sense, do i just go to the surgeon and be done with it or what? Any ideas from thise that have been there done that would be helpful.

if the box for "may be eligible for a blanket waiver" is ticked then you should be able to just provide a copy of your DS-3025 with your AOS package and it will be fine... no need to see a civil surgeon.

my DS-3025 has the same boxes ticked partly because i had my first of the 2 shots of the MMR vaccine at my medical so the form is marked as "not being time appropriate" for MMR and partly because a lot of the vaccinations are marked as "not age appropriate".

Hey Dan,

that was my error thought i was responding to an old post didnt know it would show up here. Actually the mmr is checked, retro virus is onlything unchecked, The confussing part was box checked history incomplete, and then box checked about the waiver

those 2 boxes are checked on my DS-3025 and it is fine for AOS... just include a copy of it with the package :)

Hey Kevin I scratched my head on that for days then just finally paid for the new physical. A friend her did not and got an RFE

a year ago when we did AOS, we went to the civil surgeon. some vaccinations require 2 shots, which is why it's marked insufficient time. my wife needed the second part of the shot and got it done that day. also the doctor has to transcribe the info from the DS-3025 to the I-693. you do not need another physical if it is less than one year from the one at St Lukes. you only need to complete the vaccinations.

you do not need the second shot for AOS and that is why it is marked as "insufficient time". if the DS-3025 is marked as "may be eligible for blanket waiver" then all you need to do is provide a copy of the form with your AOS package. a civil surgeon only needs to transcribe the info on to the I-693 if there have been new vaccinations since the DS-3025 was completed.

this is my DS-3025 and it is all that is needed for AOS
