I'm sure this has been covered somewhere before, but I thought I'd start my own thread about this. So, please don't get upset.

Regarding the medical interview that my fiancee is going to have, other than the vaccinations, what other documents does she need to obtain from the medical interview for when she applies for AOS? We're still debating between paying for the vaccinations here or there (I have to review my medical insurance terms), but I think Baron told me once that there was something that we needed to get a copy of at her medical interview.

Also, she told me that they'll give her an international vaccination card to keep with her. Does she only get that if she gets vaccinations, or does she get one by default? Baron, is that what you were telling me to get a copy of???

The copy mentioned by Baron is the vaccination document(immunization record), it's transcribed at the medical and you'll need to take this to a civil surgeon in the US to complete it if it hasn't been completed in home country. So at the medical in home country, ask for a copy of it.

It's the English one, called ds3025. The doctor has it. It looks like this; http://photos.state.gov/libraries/thailand/591452/iv/ds3025.pdf

The doctor has to fill it out as part of the medical regardless of getting vaccinations. Some have to ask for it, others get a copy right after it's done. If your fiancee hasn't gotten one before she's done at the medical, then have her ask for it.

Ensure she requests and gets a copy of the DS-3025. This will be needed for AOS.

Is that form used to certify her existing vaccinations, as well as any additional she begins there? I mean, if she doesn't get any additional vaccinations there, will she still need that if we are planning on getting all of the vaccinations required for immigration here in the U.S.?

I think that the G3025 is what she was referring to as the international vaccination record form, which isn't the same as her Russian vaccination card.

I called about getting the vaccinations required for immigration done here in the U.S., and they told me it'd be about $250 for the exam/consultation and all of the required vaccinations...

If you get a copy does it have to have an original signature on it, or can it just be a photocopy?

Just a copy - USCIS ends up getting the original medical. Sometimes they lose their documents though, good to have copies anyways, but a copy is sufficient when it's complete.

The medical is sent to the embassy, it's part of the sealed envelope you get from the embassy (when approved) and you hand over at POE. From there it goes to the USCIS so they have it when you're filing AOS.

OK cool. In Japan they sent it to me (sealed envelope) and also provided me with a copy of it all, so just wanted to check if I needed an original. Thanks for the reply

Yes. the vaccinations acquired before or at the medical are put into a document - that document is the link I provided.

When the vaccination record is incomplete in home country you'll need that piece paper and take it to a civil surgeon in the US - either to first get the last vaccinations and then to have them added to the vaccination record, Or go to your family doctor and get the vaccinations and proof of it(receipts) and then take that and the vaccination record to a civil surgeon and have them transcribed. A full exam isn't needed when doing AOS within a year. Keep calling until you find someone who'll just do the transcribing. It's less expensive.

Bottom line, if it's cheaper to get the shots in home country don't wait. But do indeed get a copy of the vaccination record no matter what you choose.

Congrats on visa then :)
