We got an RFE yesterday. What usually triggers these? I want to start preparing things that it may have been issued for. Is this even possible?

it's hard to tell, it could be any thing, you have a copy maybe you can look over your file to see if you missed any thing, or just wait for the hard copy, we can guess all day.

There is no way to know. It could be you forgot to sign a piece of paper or they couldn't ready your marriage cert or you sent the wrong fee or.. or.. or.... you'll have the letter in a few days and then you can spring into action.

Does an approval take a long time after the RFE?

I have seen other users who submit proofs as asked in RFE, usualy i have seen 1-2 weeks of approval if all is good.

Some even in 5 days. so I think 1-2 weeks is a safe bet.

Good luck!

I received RFE on January 12th, it was dated January 6th. I had until February 5th to return it. I sent everything USCIS requested and USCIS received it January 27th. USCIS said to allow 60 days for a decision. As of right now it will be 3 weeks tomorrow and have not heard anything from them.
