Hello everyone,

I'm filling out the forms, to send back the packet 3 to the Embassy; which I received today, by email kicking.gif

I will need to attach a page (just like it says on the instructions) for item #35 List all the previous visits to the US... there are only 3 spaces and I have way more than that...SO the question is:

On the actual form, do I put a [please see attached page] and then create that separate page with the information with the same format they have it on the form? How would you do it?

I know it's a silly question, but I rather ask my silly question than mess up

Thank you in advance for your input!

Awesome! Thank you! I made a spreadsheet with the format the form has, and listed everything. My next question is.. where you type in the Location you visited, did you use the city (or cities) you visited or the State?

City and state of the places I stayed. I didn't even make a spreadsheet (though it's a good idea), just a Word format page, named "DS-230 Additional sheet for question #35" and I listed the places like this:

"From … To …., Location: City, State, Type of Visa: …, A#: ...

From … To …., Location: City, State, Type of Visa: …, A#: ..."

and so on
