Hi people.

I have read several posts about my questions but I'm still having doubts.

We are assembling the AOS package. I have a lot of doubts regarding the DS 3025 form. My husband don't have that copy because the doctor who examined him and gave shots to him back in his country, don't lend a copy to him. How can we complete the I-693 without it?

He just have a record that shows the vaccines labels and next the nurse's signature. Is only that he needs to put aside with the I-693 Part 1? I wrote to USEM and the physician said isn't necessary. I 've been reading a lot of this topic and most said that if you don't have the DS 3025 form, you need to go to a civil surgeon and PAID for transcribe those in the I-693. He already paid in his country for get their vaccinations completed before coming here. For now, we are stuck in this step. Someone have problems with that, too? :s

Any advice?

I also never got the DS3025 in my home country but had all my vaccination records. When I got to the US I went to the CS to get them transcribed and yes you have to pay for that but its not as expensive as getting all vaccines in the US. Good luck!

He will need to take his vaccination records and have them transcribed onto an I-693 by a civil surgeon. You might need to call around to several civil surgeons until you find one that won't insist you need a whole new medical and only charge you a lower fee for the transcription only.
