im so confused about this. I talked to USCIS today and they told me that we have to take Otmans Vacccination suppliment to a civil sergent here and then they varify and fill out the I-693

all we have from morocco is a vaccination book with the listing of the shots he got.

what happened for anyone else? what info did you have and where did you go?

im stressing so much now about this..ugggggggg.

We found a civil surgeon that would take the information from the Vaccination worksheet and put it on the Vaccination Supplement. He charged us $35.00 for this.

No medical exam is necessary as long as it has been less than one year since your medical exam in your foreign country. Call around to find a civil surgeon that will not make you do the medical exam over and that will just prepare the Vaccination Supplement.

well you are farther ahead in the game if you have the worksheet, you can find a civil surgeon that will fill it for a fee. Adnan couldn't get that supplement from the doctor in morocco so he had to get all the shots again from a doctor here. it sucked.

so call around and find a civil surgeon that will fill the 693 for you. I had to call a lot just to find someone who would do the shots without the complete new medical exam.

We are having the supplement filled out by a civil surgeon for $25. Abderrafie has a copy of the worksheet. I believe they are drawing blood to make sure the vaccines are present in his blood but will know for sure when we go tonight. They said it would take 5 days for the paperwork to be complete.

I found it helpful to call several. Some insisted on doing a physical as well. There were various turnaround times, and the fees greatly varied - even with different locations of the same medical group.

I already answered you in another post but just saying that the civil surgeon we went to charged $50 to complete the form. I'm sure you should be ok and he could just list the shots for you on the form. Then you will be set to mail it off with your paperwork. We called and they took him in the same day and gave us the completed form in a sealed envelope.

So for Youssef's vaccine supplement I have a question. He doesn't have a print-out listing the shots he does have but he has a sheet (based on the required vaccines) of shots he DOESN'T have. So do you think they will take that at face value and sign off on it? The paper is from a doctor and it clearly shows that on the paper.....I'm just wondering?

It couldn't hurt to try. If there are several civil surgeons in your area, it would be worth calling around because their policies vary.

We have a health care group here in Maryland. I called two offices of the same group and they each had different policies. The one said a blood draw to verify the vaccinations was mandatory. The other office (which we went to tonight) did not draw blood and is filling the paper out based on his vaccination worksheet alone.

We had the supplement from Morocco, so we took it to the local health department, got the rest of the shots he could get for now, and they did the I-693 vaccination supplement for us there. Then we just had to take that to the civil surgeon who surprised us by signing off on it for free.
