Just want to clarify what I have been reading... we need a civil surgeon to fill out the vaccine document? I thought that the US embassy doctor filled all of that out in the beneficiary's country? What's the real story. He is from France and has a list of his vaccines... and does anyone know if this costs money to have the information transferred ( I'm sure it does). How long does this take to get done? Any help would be appreciated.

if the DS-3025 is marked as complete then there is no need to see a civil surgeon in the US. if he had all the required vaccinations before the medical then you are fine for AOS

Even if it is marked incomplete he doesn't need to have it transcribed. Because of age requirements of some of the vaccines some of them are not required for everyone. All he needs to do is send a copy of the DS-3025 with the paperwork. Unless the medical is over a year old.

Yes it will most likely say that. He still can just send it in the way it is....Not the original, just a copy. Keep all originals for your records.

Another question about the medical for AOS.. The medical information was in his packet that he gave to the border officer and he doesn't have a copy of his vaccines, the embassy has this. What should we do?

Hi, we have almost completed the AOS package for k1 visa, but are confused by the necessary medical documents.

My fiance gave the customs agent a brown envelope, given to him by the US embassy in Paris. This envelope had the approved

medical documents. He has his radiograph, but not the vaccine info as that was in the brown envelope ( where does that end up btw???) Any help thoguhst etc.

Also, he has been here for 1 month, we are married and want to file the AOS soon.

if he has his ds form, he can use that if vaccines are recorded on it. If not, he needs to go to a civil surgeon and have his immunizations entered, and get any shots started he is missing.

He can go to a primary care physician, and get blood work, and shots plus titre test (blood work to see which immunizations are administered) done, plus TB skin test, then take those records to a civil surgeon to save time and money.

get a copy of vaccines from the medical examiner in hte country he came from or his doctor in that country.

If the DS-3025 is marked as incomplete, it has to be filled out by a civil surgeon, but not the rest of the form IF and ONLY IF the medical for the embassy was under ONE year. A titre test of bloodwork will show if the vaccines needed are present or not. Most if not all of the vaccines on the I-693 should be able to be tested for.

You just have to show you are starting the regimen of getting the shots, you do not have ot have completed the shots in order to file. However, by the time of hte interview, most if not all shot regimens should be completed. Unless a waiver has been given, they can place a hold on issuing GC based on completing the vaccination requirements.

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.

See earlier replies in this thread. Even if it is marked incomplete, if they had all the required vaccinations at the time of their medical when the DS-3025 was filled out, then they still do not need to see a CS.
