We are about to head out to mail our AoS packet, but I have one lingering question.

The guide states "G-325A filled out, signed and dated" is that a single one for me? Or one for my husband too? I'm a little confused by only that part of the guide =\

For AOS the beneficiary is the only one who has to submit/file a G-325a.

One final question - My vaccination sheet I got from the medical for K1 application say's "Vaccination History Incomplete - Applicant may be eligible for blanket waiver because vaccinations not medically appropriate" and the checked boxes are "Not age appropriate" above.

I do, however, have one "insufficient time interval" for my MMR with a note saying "4 weeks". I have a receipt from the doctors surgery stating that I've paid for the second MMR vaccine and of course it it dated. Can I submit that alongside that sheet, or will I have to have a civil surgeon fill I-693?

I also have a copy of my entire vaccination history from my previous practitioner.

You can summit it. No need to go for civil sesurgon as long as your K1 medical had done within a year. My vaccination also have checked boxes as incomplete but it was okay. I didn't get any RFE or question from officer at the interview.

One MMR is enough if it's recorded on your DS-3025. Insufficient time interval is a reason you can get the blanket waiver meaning excused forever. They don't accept receipts or dates from anybody but a panel physician abroad or a US civil surgeon. What your GP writes doesn't count except to show a civil surgeon so he can sign off on it.

Nothing for immigration. If you ever want to know what your lungs looked like in 2011 you can break it out and take a look.

I know this is an older thread, but is there anything that must be submitted to get that "blanket waiver" for the second MMR shot? When Paul went for the USCIS medical in London he had the first shot and his DS-3025 is marked "insufficient time interval" for the second one.
