Hello my husband had his medical done overseas he only received his x-ray film is this the Ds-3025, if so do i call a civil surgeon and have him transfer this information to a I-693 form? also he got all of his vaccinations except one in mexico, do i also provide the civil surgeon with his vaccination record? I am really confused so any advice will help! my husband entered on a k-1 visa if needed.

DS- 3025 is NOT an Xray - an Xray is an Xray..

DS - 3025 is a copy of the vaccination sheet that you get at the medical - sometimes they just give them and sometimes you have to ask for them.

If he is missing shots he needs to get them - then have the 3025 transcribed to the 693 with the updated shots.

Where did he get his vaccinations in Mexico? Does he have a vaccination record to prove he got them? Juarez doesn't give vaccinations for K-1 applicants at the medical and they tend to not to bother with the DS-3025 at all, so it is most likely that your husband never got that vaccination document. He will need to bring his vaccination record to a civil surgeon and get them to transcribe those to the I-693. He does not need a whole new medical exam, but you said he is still missing one vaccination. He will need to get that from your family doctor, the health dept. or the civil surgeon, and have his vaccinations all transcribed and showing as completed on the I-693.

In your OP you say he is missing a shot. Because of this yes he needs to see a CS to get the missing shot and have that and his DS-3025 transcribed into his I-693.

They won't give you a DS-3025, and if they do, it will be blank because they didn't give any vaccinations and won't transcribe the ones you have onto a DS-3025.

He needs to take his vaccination records from getting his shots in Mexico to a civil surgeon to have those transcribed onto the I-693. The civil surgeon can tell him which one/ones are missing.
