Ohh I see, so then in the end I just submit an I-693 and not the DS-3025? That's brilliant helps a lot.. I looked up Civil Surgeons and there's one only 10 miles from here.

call them to schedule a visit and check about prices. Some will try to give you an entire new medical exam (about $300, leave or take), instead of just rewrite the Ds-3025 results into certain sections of I-693 (about $50, compiling job done by a secretary, you won't even see the doctor, time required to complete the task: 3 minutes. A total ripoff, IMHO). Also, make sure the secretary seals the envelope in which she places the form, you will have to submit the sealed envelope with the AOS package.

Your wife can sign with her married name. You may want to go to SSa first to change her name before getting a new license as the DM in some states will not change the DL if it hasn't been changed in the social Security database.
