"c/o means care of, used when there is an intermediary that will distribute your letter or package",

so care of would only be necessary if someone else was to receive your mail before passing it onto you

it means if you live with someone else or if your sending it to an address that is not your own you put that name for the address usually it will look like this :

C/O Allisson Moore

555 someplace street

If you just live at home don't use it. It is also left blank on the example forms on VJ - http://www.visajourney.com/examples/N-400.pdf
