My husband just received his package, should there be a form that he takes with him for his medical? There was just a list of the doctors, no form.

My husband didnt recieve a form either. He called the doctor, Khadija something, and asked her about it. She said that she had the forms there so dont worry. The doctor sends the information to the embassy directly and doesnt give anything to your husband. The exam was easy, just xray, two injections and some questions. Several times she asked my husband is he smoked hashish. He said of course not and everything was fine.

my husband also got the form at the doctor's office since we did DCF. i think the doctor's that do the examinations must just keep them at their offices. i wouldn't worry about it. you can always ask when you call for an appointment, that's what we did :)
