I have posted this also in our own Feb forum, but since you guys are a little ahead of us, I wanted to ask your opinion

My fiancee went to her medical appointment today and will pick the result up on Saturday. When she asked the doctor about a copy of her medical result (form DS-3025), no matter how hard she insisted, they refused to give her a copy and said that they will only give one copy for the embassy which would be in the sealed envelope. Maybe I am a little confused, but I have read that in order to avoid going to the medical check again in the US for AOS, we need a copy of that form. If this is true, our only chance is to ask the embassy to give her a copy of the DS-3025 form when they open the envelope, however don't know whether USCIS will accept that copy or it should have an original signature! Do you guys have any opinion or experience in this regard?

don't we get the WHOLE PACKET back.. after they go through the POE?? so you would then have copy.. yes/?

You will be given a copy of your immunization record which is the DS-3025. You will get this separated in a small envelope and ur medical to take to the consulate. I hope u get a copy of this.

No, you DO NOT get anything from the packet at POE. Your basially a courier from the embassy taking documents for USCIS.

no they keep it! To my understanding it should go back into USCIS files for record.

the funny thing about that packet is that it has everything you submitted to the embassy. all the forms and stuff you took to the interview. my envelope had a small opening at the top so we could peep. DO NOT TRY OPENING THE ENVELOPE. thats the most important thing

Javi's visa was approved today! Super easy interview, we got inside the embassy at 7 and were out by 1030, the interview itself was only about 5 minutes... I will do a complete review when I have my computer

Sara is right. The sealed package that you give to the POE officer will be sent to the USCIS. djricky is also right! It contains ALL the documents submitted before and all the forms issued by the embassy. Apparently, from what I have read on VJ, somewhere between POE and USCIS some of the medical results (Form DS-3025) may "get lost"!!! Several people have been given RFE after filing their AOS for not having medical results on file! Therefore, the advice from VJ and all those people is to try and get a photocopy of the form from the physician and submit it with your AOS package. I told my fiancee to ask the embassy to give her a copy of that form after they open it, hope they accept to do it

Javi's visa was approved today! Super easy interview, we got inside the embassy at 7 and were out by 1030, the interview itself was only about 5 minutes... I will do a complete review when I have my computer

Ditto!!! Haha. We were out a little while before Javi and Kate, but yea, the whole interview was super fast and easy. The waiting before hand was killer!!! We waited foreverrrrrrrr just to turn in the paperwork before we could wait for the interview. Only saw one guy leave crying...everyone else seemed happy coming out. Anyway, we had a nice brunch all of us afterwards to celebrate! Haha. Then we all came back and napped at the hotel for hours....that 4:30 am alarm was killer! So yea, visa pick up tomorrow afternoon and then heading home to pack and get ready for the big move!!!

No it is not true. You should ask for a copy only in case they lose it (which happens often). If they didn't give you a copy and you have the medical done within 1 year then you don't have to repeat it. However if they lose your results, they might ask you to go to the doctor with your vaccination record and he will have to fill in a I 693 form and you would have to send it to the USCIS. That's why it's good to have a copy but not required so do not stress

BTW DS-3025 is just a vaccine supplement and not the whole medical. Does it mean the doctor didn't want to give her that supplement or the copy of the whole medical? I got both but as far as I know, they should at least give you DS3025 unless Canadian doctors work in a totally different way...

And copy is just a copy, do not worry about the original signature on this occasion
