DS-160 question and 1-134 question

Ok, I realize these questions have probably been asked a million times in the past but I can't seem to find the answers anywhere. If I do find answers, there are many contradicting ones. So I was hoping someone could help me with the following questions:

1) DS-160, looks like it needs to be filed electronically and it looks like the passport photos need to be scanned in. Is this correct? What if the scanning doesn't work correctly can you submit the photos with the rest of packet 3?

2) For the 1-134 what are the exact supporting documents that they are looking for. I have read that you need bank letters but have also read that you don't. Which is correct. Also,how many years do you need to go back for w2's and tax forms? Finally, on the 1-134 itself if you have stocks, do you need to list of individual purchase prices or just the total number of shares and their market value?

Thanks so much. I hope someone can help clear this up. The NOA2 is on the horizon and I am trying to wrap my head around the rest of the process.

Aos:

10-01-12. Sent AOS, AP and EAD

10-09-12- Recieved NOA's for forms

10-24-12- Successfully walked in for biometrics (originally scheduled for 11-08-12)

11-26-12- Made info pass appointment for emergency advance parole. Denied, but agent called service center to request an expedite.

11-28-12- Agent at Infopass called me to let me know service center approved my request.

12-01-12- Advance parole received in the mail (2 page version, not combo card)

1. Print off the DS 160 when you fill it out (it'll give you the option at the end) because Montreal doesn't have the electronic system completely set up yet. Not sure about the photos but it's probably important to include them on the DS 160. Also make sure that the bar code prints out clearly.

2. The instructions for the I-134 indicate what evidence is required. Bank letters are optional, as are employer letters, but it's a good idea to obtain them if possible. 3 years worth of tax returns or tax transcripts is the standard recommendation although the most recent one is the most important. It really depends on what kind of financial shape the USC is in. I recommend at least bringing 3 years worth of tax stuff, though. At my interview they kept copies of my husband's most recent tax return along with the employer letter, bank letter, and a printout showing his 401k. If you don't go with transcripts make sure to photocopy his W2s and returns since Montreal will want to keep them. For the stocks I don't know.

We had 3 years of tax returns, a letter from his employer stating his wage + gross income this year so far + "no changes to employment in the foreseeable future" statement, and recent pay stubs.

I don't believe you need any "proof" of stocks. It asks you for assets on the I-134 form where you list houses/stocks/cars etc. but we were never asked for any proof of the value of those items. They didn't ask anything about the Affidavit at my interview so I assumed it was all in order.

The DS160 - we are talking Montreal? The DS160 is done online and you can try to print each page as you fll it out. Lots of people have difficulty getting every page to print. I did. But it didn't matter cause they receive it electronically. Also many people have difficulty with getting a picture to upload. I got a fresh picture taken at the drugstore (american passport size 2x2) and mailed it when with the DS160 CONFIRMATION page that you print off after finishing submitting the DS160 electronically. Print the confirmation, staple your passport picture to that, and mail that with your packet three. Good luck!

I did not attempt to scan in a picture for the DS-160. I just attached the passport style photos with my confirmation page (wasn't able to get the whole form to print, either) and sent it in with Packet 3.
