No issues at all. Submit the Original Birth certificate that is what they require. They really are interested in your information and will use monther/father's name to establish relationship/family tree.

Well said RICARDO4EVA2

And even if you hear this question on your interview, you can explain it to them. The visa counselor's are way too qualified and understands the local rules, laws and social norms. The worst case they ask you to make correction ((which I doubt will happen), you can do that as well.

Just don't sweat over this and it won't be a big deal at all

Is your passport going to expire within the next few days or expired already? If not, you should send the current one. In case this gets expired between now and the interview and you get a new one, that should have some indication about the last expired passport and you can always staple the old copy at the back of the new one.

Normally, it is safe to apply for a visa with a passport that still has six or more months before expiration. If your passport is going to expire soon, you should be able to renew it. Not too sure what the requirement is in your country, but in most cases, you should be able to have your passport renewed a few months before the actual expiration date. So I'll say follow up with your country's passport office to find out the requirement.
