oh wow that sounds so confusing. plus my paper says "for use with DS-2053 or 2054" so i think is not the same thing as you guys are saying but my list looks exacly like the one Harpa Timsah posted

Check the column that says "Insufficient Time Interval" on your DS-3025 - if there are any checks under that column, you need the next dose for those vaccines. In my case, I had to finish the vaccination series for Tdap, HPV and Varicella. You can get them done at any clinic or your local health department but only a Civil Surgeon can fill out the I-693.

If you need more vaccines, you will need to have the vaccination info on your DS-3025 as well as the vaccines you got in the U.S. transcribed onto the I-693 form. Some Civil Surgeons may charge just $10-25 for it but the CS we went to insisted on charging us the price of a full medical exam even though I didn't need one (it was over $100 :()

*** Back when we sent our AOS packet, we did hear of some people getting RFEs for just sending their DS-3025 but I think they've changed the I-693 instructions???

Yes, the TD/Tdap booster only needs to be given every 10 years, you were marked off as having chickenpox and HPV is not required. If your MMR was complete, then I think you are good to just send in a copy of your DS-3025 without seeing a CS.

ok so we're almost done with the AOS package but i have a small problem with the vaccines. before the interview, the doctor who checked me gave me a copy of the vaccines list, told me to use it when i apply for aos. i dont really understand this thing. there are 14 boxes with vaccines. 5 are "completed series", 2 are "not required", 6 are "not age appropriate" and 1 "insufficient time interval" (i have 2 shots done in that category). the results are "vaccine history incomplete" and "eligible for blanket waiver". so do i need to see a civil surgeon in this case? to me it doesnt seem i need other vaccines and i have the xrays from the interview from september :blink:

Everything you have described indicates you do not need to see a civil surgeon. You can be given a (blanket) waiver for a particular shot for these reasons--

  • "Not age appropriate"
  • "Insufficient time interval" (Some immunizations are a series of 2-3 shots with a wait time like 4-8 weeks in between. It wasn't appropriate to get shot #2 yet on the date the form was signed so you are excused as long as you got shot #1)
  • "Contraindicated" (means getting that shot might be bad for you if you have proven allergies to it, or getting MMR is bad for pregnant women. Also excused.)
  • "Not flu season" (on the date it was signed if it wasn't between Oct 1- Mar 31)
  • "Not readily available" (This doesn't work for K1s because you can get the shot after arriving in the US. It works for CR1s who basically have their "AOS" requirements done before they arrive. No way to get the shot in their country, so CR1s are excused.)

"Blanket waiver" means a requirement the USCIS adjudicator can excuse without a waiver form filed by you. Almost all DS-3025 forms will be completed with "MAY be eligible for blanket waiver". It doesn't mean your requirements are complete. If you didn't have MMR, for example, and no waiver reason ticked why, then you would not meet the requirements for AOS immunizations. You basically need a shot date or a waiver reason ticked.

To the OP--Did you actually get at least one date marked by these??

Then I think you have completed the necessary vaccines by more than is even required for AOS. I would skip the civil surgeon and use the "complete" DS-3025 as my proof of compliance of the vaccination requirements. A flu shot should have been given if the exam was since Oct 1. But if flu season is over (Mar 31) by the time they finally approve your case, they will waive that shot too. So hope the AOS isn't approved until April. That's the only possible problem with your shots that I see and it may not be a problem at all because AOS takes longer than 2 months to get to the approval point. I'd wait and not spend money with a civil surgeon until USCIS said I had to in an RFE.

As always, it's your choice.

i had my medical check on august 23rd so im safe without the flu vaccine (IF they didnt lose my file LOL) thanx so much everyone we'll give it a go. feel way more relieved now.
