ok so we're almost done with the AOS package but i have a small problem with the vaccines. before the interview, the doctor who checked me gave me a copy of the vaccines list, told me to use it when i apply for aos. i dont really understand this thing. there are 14 boxes with vaccines. 5 are "completed series", 2 are "not required", 6 are "not age appropriate" and 1 "insufficient time interval" (i have 2 shots done in that category). the results are "vaccine history incomplete" and "eligible for blanket waiver". so do i need to see a civil surgeon in this case? to me it doesnt seem i need other vaccines and i have the xrays from the interview from september

Is that category for hepatitis?

Hepatitis requires shots done at specific time intervals so you will need to figure out if it is time for the 3rd shot. I do not know if it is required for AOS.

I believe "eligible for blanket waiver" means that your vaccines are complete. You could always try it and see what happens, the only risk is an RFE. Good luck.

Yes you need to go to a Civil surgeon to xfer Vaccination record to the I-693. Call around because they will try to screw you and tell you need a complete physical in which you don't. You may not be up to date with your vaccinations and if you have private insurance go to a regular doctor to get your shots before you see the civil surgeon. alot of times your insurance will cover the shots, mine did. I ended up paying the Civil S. 26 dollars per applicant , I did 3 of them. Some out there wanted to charge me 1100 bucks, don't fall for that.

 Yes, read that post. I do not think you need to see a civil surgeon. Just send in a copy of your DS-3025.

If you still want to see one anyway, be sure you find one that will not insist on you getting a whole new physical and charging you an outrageous fee. You can get your shots wherever you are covered on your health insurance. Just take the DS-3025 and a record of the new vaccines you get with you to the CS.

Thats incorrect , the Ds-3025 is always filled out in Country of origin, that does not mean your complete(you could be missing certain shots) and thus needs the signature of a Civil Surgeon to assure Uscis that your up to date with your shots, its a requirement for the green card.

"I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. Only if your DS-3025 is marked "incomplete" is a I-693 required to be completed. If it is marked as such (incomplete) you are required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US. If you do not have a copy of your DS-3025 you will need to provide sufficient evidence of your vaccination history to the Civil Surgeon (talk to the civil surgeon's office to discuss what vaccination records they accept as proof). If you do not have proof of your vaccination history the Civil Surgeon may insist on re-administering all the required vaccinations prior to completing the I-693. Once the Civil Surgeon has has verified that all required vaccinations have been performed, they will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon's Certification). They will then seal the form in an envelope and return it to you." (this is from the guide for AOS here on the site)

In my case they did check the copy of my DS-3025 to see if something is missing.I did need a vaccin and aftre they did transfer the information from DS-3025 plus the new vaccin on the I-693 form.I think if u no need a new vaccin(or more), they will just complete your I-693 with the information from DS-3025.I also think that I-693 is necessary when to apply for AOS :)

If u no need a vaccin, the process doesnt cost much. And if u have insurance it might help u,because when i was they ask me if i have, but i didnt at that time.

I know where the DS-3025 is filled out. Sometimes an individual may still be in need of more vaccinations, and sometimes they may not, even if it is marked "incomplete" and "may be eligible for blanket waiver". If all your vaccinations were complete then, there is no reason to get a transcription. That would be a pointless waste of money. The I-693 is only required if you are missing any of the vaccinations that are required for AOS at the time you had your medical and the DS-3025 was filled out by the panel physician. If you had all the vaccinations complete, then you do not need to see a civil surgeon. If you need/needed to get more vaccinations in the US, then yes, you would need to see a civil surgeon.

That post by Nich-Nick explained it all very well. Please it read it. Many people have sent in only a copy of their DS-3025 that is marked as "incomplete" and "may be eligible for blanket waiver" and have been approved without an RFE because they already had all the vaccinations they needed.

Just so you know, the guide is outdated. A new version of the I-693 form was put out on 11/16/11. The form is different and so are the instructions for the form. People should always check the USCIS site for the newest version of the forms and instructions. The attachment includes the most up-to-date instructions for the I-693.

Also, if you do not need anymore vaccinations, there is no need to see and pay a civil surgeon. Unless of course you do not have a DS-3025 or it was not completed and filled out properly by the panel physician.

My wife and 2 kids needed more Vaccinations, thanks for the clarification.
