Requirements:

- 5 year legal permanent resident (this is based on your green card "resident since" date, not the date you came to the US).

- Physical presence: At least 30 months in the US for the last five years.

- Continuous residence: No single international trip lasting longer than 6 months

- At the time of application: Having lived in the same USCIS district/state for the last 3 months.

If you satisfy the above, you can send your application 90 days prior to your 5 year anniversary (use the "residence since" date on your green card). Be careful, do not apply 3 months early, it needs to be 90 days. You can use the USCIS date calculator found here: http://www.uscis.gov/dateCalculator.html

Read also all the instructions for N-400 found here: http://www.uscis.gov/files/form/n-400instr.pdf

There is also a document check list found here: http://www.uscis.gov/files/article/attachments.pdf
