As I was reviewing our packets, I reviewed our copy of the medical form. The civil surgeon's office did not put the entire date because the form wouldn't allow them. So there are dates with month and day, but no year. REALLY? Are they that incompetent to think USCIS will accpet this? Did we not pay for a service that we expected to completed in a professinal manner? I think we did. Now I will go back to that office and fight with them to hand write the dates since the form is not allowing the dates to be entered correctly electronically.

Has anyone has this issue and if so, how did you handle it? I can't see how USCUS would accept that kind of work from the civil surgeon on such an important, legal document.

You should go back the office and had it filled manually, no need computer typed. A RFE will slow down everything for such a silly mistake of someone.

Well in doctor's office you should get 2 envelopes: 1 for your records and 1 for USCIS sealed that might say "do not open".

So now the question is did you open the sealed envelope the one meant for USCIS?

PS: There are good doctors and "crooked" doctors, hoping you to receive RFE for incomplete Medical form and charge again, also no matter what TB skin test might show, they will make to take x-ray test in their office, you can always opt-out and say you can do it somewhere cheaper or even free. Same goes for shots.

We did get two and no I did not open the sealed one. I told my husband that the incomplete date would end up with an RFE. Hence the reason I am going back to have it fixed. Glad I noticed it before we sent it in. Hopefully the office won't give me too many troubles in filling it out manually. Never had an issue years ago when I went through USCIS on another case.

Dillegence is required every step of the way!
