AOS for K1 civil surgeon?

Hey guys, thank you all so much for all the information that has helped me and my fiance be togethere here in NY. We have started getting the paperwork together for the AOS and I am confused about the I-693. I know we do not need another physical exam because she had hers in Santo Domingo 2 months ago. However I am reading different opinions if I need a civil surgeon to complete a I-693 for the vaccinations or if I can skip this process and just send in a copy of my 3025 Vaccination report that i recieved in santo domingo. If any other k1 visas from the DR went through this please let me know. Id like to skip paying a civil surgeon if it is not necessary however I do not want any RFE's on the AOS . Thank you all. And BTW if i do need to see a civil surgeon does anyone know of any in the Long Island/Queens area? thanks so much!!!

If you had all the required shots at the time of your medical, then you do not need a civil surgeon. Even if the form is checked "incomplete", if it is also stated that you have a blanket waiver and you had the ones that are needed for AOS: MMR, tDap and varicella, then you can most likely still skip the CS. If you are still in need of getting vaccinations, or received some vaccinations already since you arrived in the US, then you need to see a CS and bring your DS 3025 and any other vaccination records you have so they can transcribe them onto an I-693.

I also had to check on this a while back. What Jay-Kay says is correct. If you've already had ALL of the vaccines, you won't need those done again. But if you still need some done or you have had some while you've been here, they those need to be transcribes...and that can be a little pricey....so shop around if you end up having to do this. I live in the Boston area...but when I checked it was around $300 for the transcribing.

Check around for the Civil Surgeons in your area and see how much they'll charge for the I-693 transcription. I've heard some who got it done for around $25 but the CS closest to us wouldn't transcribe my vaccination record onto the I-693 without doing a physical exam - even when my husband explained the situation, the CS still insisted on charging us the full amount, over $100 ;( (without a physical)

You do not need another physical as long as it has not been more than a year from the last physical. Since my fiance's vaccination report from DR stated incomplete I went in to a civil sureon to make sure he had all he needed and avoid any mishaps with USCIS lol. My fiance had to get the 2nd shot in the 3rd series of Hep B and some other shots but we went to the NY Dept of Health and got the vaccines for free. Also we went to a civil surgeon in the bronx to transcribe the vaccines and get his signature and paid $50 :)
