Does she have a DS-3025 from her medical? If so she can submit that. If not she will need the first and last page of the I-693 (immunizations) signed by a Civil Surgeon.

I will be filing aos for my wife,she is on k1visa,she had her medical examination recently in her home country for her visa interview,do we still need to fill form I-693

Where her shots completed at the time of the K1 medical?

Yes,shots were given during her k1 medical,and medical report n record was submitted to US Consulate,India

Okay well if her shots were completed at the time of the k1 medical then she doesn't need an I-693, you should just be able to send a copy of the DS-3025
