California Civil Surgeon..How much it cost???

Hello everyone,

Is anyone here knows a Civil Surgeon from Southern California who charge less for vaccination supplement I-693A?Please endorse me with that kind of Doctors..All I need is to transfer the form since I already completed my Vaccination and I have the copy with me..Thank you.

Hi! I am from North Hollywood, California, and I went to Sherman Way and completed my I-693A. There you can see a Vietnamese civil surgeon. I paid a total of $170 for TD vaccine(anti-tetanus shot because it's not available in the Philippines- and it is required) and blood test for HIV and syhpillis tests. :yes:

I gave him the vaccination worksheet from St. Lukes Clinic and Xray film, and he transferred information to the I-693A form. :thumbs: It took 3 business days to complete the tests(HIV and syphillis).

Where in SoCal do you live? :whistle:

June 30, 2005- Sent I-129F

July 12, 2005- Processing Date

September 16, 2005- I-129F Approval Notice

September 29, 2005- Approval Notice sent to US Embassy in the Philippines

January 26, 2006- US Embassy sent Medical and Visa Appointment dates

March 2, 2006- Medical Appointment

March 9, 2006- Visa Interview

March 21, 2006- Visa received

March 24, 2006- Attended CFO Seminar in Q.C.

March 28, 2006- Flight to the USA

Marriage:

May 1, 2006- Applied for Marriage License

May 20, 2006- Got married in Chandler, Arizona

I-485/I-765 Applications:

July 24, 2006- I-485/I-765 sent to USCIS

July 31, 2006- Processing Date

August 2, 2006- USCIS sent a Biometrics Appointment Letter

August 8, 2006- USCIS sent an Appointment for AOS

August 16, 2006- Biometrics Appointment

October 3, 2006- AOS Interview(Los Angeles District Office)

October 10, 2006- Received Welcome Notice, Employment Authorization Card

October 18, 2006- Greencard received

I-751 Application:

August 29, 2008 - I-751 sent to USCIS

September 2, 2008 - Processing Date

January 2, 2009 - Biometrics 1

April 6, 2009 - Biometrics 2

April 7, 2009 - Biometrics 3 (never ending)

April 22, 2009 - Received Approval Letter

June 16, 2009 - Greencard received

Paramount,CA..I think the blood test for HIV & Syphilis is already done in St.Luke..What i need is the TD vaccinne..By the way,how many shots you get with the MMR?Thank You.

We went to a CS in Santa Ana and between the tetanus shot and filling out the form I believe they charged me $110. I suggest you find as many CS around your area as possible and call around because the cost difference can be a lot.

I just had my wife finish her Medical. Go to the USCIS website and find a Civil Surgeon in your area. We went to a Industrial Medical Clinic (basic work physicals) and paid 100.00 for the exam and 50.00 for chest X-Rays for a total of $150.00. I think if you can get to an Industrial Medical Clinic it should be less expensive. We had to wait 3 days for the blood work to finish and have the TB test read. We also got the Supplemental Work Sheet finished. This clinic had all the forms, but it will not hurt you if you printed off all of the forms you need and bring them with you. I just felt better bringing my own, but I didn't need them.

We are looking up the same info.

I did a seach by zip code on the USCIS search and they came up with four DR's. then I called them and found one we could work with. $80 for the Physical and $30 for the lab work.
