Confused about I-693

I went to see a civil surgeon and was given an order to get a X-Ray done, since they don't have the machine that reads my X-Ray CD. The nurse advised that in many cases a person that had just been given the vaccinations I had would come out positive to the test. I thought to myself that was a good and cautionary advice, and I could also get the X-ray done using my health insurance. Now I got the I-693 and on the blank spot under page 2 of the Civil Surgeon Worksheet item 1. Tuberculin Skin Test (TST), the doctor wrote "Positive PPD in the past." And on Item 3, he check-marked chest X-ray required due to initial screening test results. Then on Item 4, he check-marked "No Class A or Class B TB." I was worried and asked the nurse why it said "PPD positive in the past" when I never was. She said it was OK and wouldn't be an issue. Now I am ready to send off my packet, but so confused and stressing over should I include the I-693 or not??? I really need your advice on this, VJ...Hopefully, I'll be able to read some advice in the morning. Thank you very much!

goto a different civil surgeon - the one you went to , is not so bright.

You went to the wrong civil surgeon. You need to find a civil surgeon who knows the drill. All they need to do is to transcribe your DS-3025 onto your I-693. We paid a civil surgeon $35 for her transcription service and we were out of there in 15 minutes.

the problem is most civil surgeons out there just want to rip everyone claiming they are following the USCIS protocol..I hope there are ways to contact the USCIS about this practices of civil surgeons- allt hey do is just add up complications to the whole process which should NOT be - just sayin'
