Arooj I too am having the same problem with my fiancee. The only thing is that she doesn`t have any of her vaccination records since her parents lost it a long time ago. The doctor did not issue her a vaccine since he said that it wasn`t necessary for a K1 visa. Sorry just needed to rent a little :-/ I will be following your thread

Oh dear what a bummerrrr!!!! i'm sorry to hear that Erik....did ur fiancee get her medical done from Hameed Latif Hospital in lahore?? cuz those morons dont know how to do their job right.... only IF they would've given us that one piece of paper with the vaccination record we wouldn't have to go thru all this trouble.......my birth record of vaccinations which my parents thought was long lost was luckily found in my grandpas briefcase and since it was in urdu i took it to a doctor n he transferred the records n the vaccinations i had gotten later on a new card so i'm hopin' i can use that...i did ask my dad to send me a scanned copy of it...i'll take that along too just in case....n omg beware of these doctors trying to rip u off by saying u need a full medical if they say that don't listen n read them the info thats on the USCIS website link posted by Zee148 cuz u can get that done for a fraction of a cost as compared to what they tell u....but good luck with AOS you two.

OMG i think those doctors were trying to rip me off by saying they don't fill out the I-693 without doing the full medical exam n they just wanna make money by charging $225 per medical exam.... cuz even the USCIS rep. told me i just need parts 1, 2 n 5 filled out. So it makes no sense in doing the whole medical exam if i got it done within the last year and it says excatly the same thing on the USCIS link you posted zee481....but ugh..i already made an apppointment with this doctor in Milwaukee, Wisconsin... n the lady from their office was like its gonna be a full medical n i have to come back 2 days later to get the tetanus exam checked n to pick up the results as they need two days for the blood work to come back n she said to bring $225 ..pffttt!!! ...so guys what do u say i should do ...should i call 'em back n ask why they cant do it seperately n for 1/3 of the price of the medical cuz in those threads those people got theirs filled out for under $30....OR should i just go n take a print out of the USCIS page where it states that about vaccination supplement filled out seperately, n show it to them if they dont budge??? this is really frustrating as me n my husband really dont wanna pay that much because we're in a tight spot financially.....

yeah you need to tell that doctor that you don't need full medical as per USCIS instructions. tell them that you spoke to USCIS and its on their website. You should also try contacting other USCIS authorised civil surgeons in your area. when you call them, just tell that you want vaccination part to be completed and you have all of your vaccination history.

you are very lucky that you found your record. i never asked my dad about my record as i thought there is no record system in pakistan lol so i took necessary vaccines for immigration again and it cost me 87 pounds.

haha yeah strangely they do have a record system but its really messed up...they've upgraded it a bit now but since i was born in the 80's...i dunno what they were thinkin back then...the card only has like 5 or 6 columns n only shows lie 4 or 5 required vaccines as compared to the 10 or so mandatory vacccines... i'm glad my parents kept me updated on my vaccines tho

I don't think your vaccination record can save you from getting extra vaccines as i can see in my DS-3025 that vaccine Tdap need to taken after every 10 years. I will still motivate you to try Islamabad embassy call them and ask them to send you your form here in USA. May be this can save you few $$$ n time.

yeah you are right...things are upgrading over there like i was surprised to see my computerised Birth Certificate last year. i was also born in 80's n i remember i used Form-B and Metric Certificate as my birth certificate lol

regarding mandatory vaccination for this process, you only need vaccines for

MMR

Td or Tdap or DT or DTP or DtaP (One no longer than 10 years ago, ie.have a booster)

Varicella or VH written by it if you had chickenpox

Influenza

at least one shot of a series.

lol really cos you could have asked them to give a copy but as i said try ISB.

tdap is basically combined Tetanus, Diphtheria and Pertussis Vaccines. when i did my medical here in london, they asked me if i have vaccination record. i didn't have any but i told them that i had chickenpox, so she marked VH for varicella n didn't give me any vaccine for it..i took one shot of MMR, tdap and flu vaccines.

i think you still need to get tdap n flu vaccine as when you go to a civil surgeon for an I-693 you are resetting the medical exam date for immunizations and you may have to get second doses of shots or a flu shot to become current on your new immunization date of record..in your case tdap.

Also you need to take flu shots as you are filling your i-693 form between Oct 1 and March 31. so no good news lol
