vaccination supplement for Adjustment of status after K1

Hi guys...i have a question regarding the vaccination supplement required for the adjustment of status form

......ok i came to the US on a k1 visa and got married to my husband on nov 12th 2011... n now i'm in the process

of filing for my adjustment of status.... So, on the FORM I-485 for adjustment of status...it says that instead of

a medical report it requires the applicants to send in just their vaccination supplement IF they had their medical exam

done within the past year......Now....the problem with my case is...i do not have the vaccination supplement...i was

not provided with it with my medical exam which i got done in Pakistan.....So, now what am i suppose to do? Should i just get

a medical exam done from an authorized doctor here or what? Oh and i also called a couple of USCIS doctors n they said that they dont

just fill the vaccination supplement form n that they only do complete exams....So, what do you people suggest i should do? Please

reply as i'm very close to my deadline...thanks

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.

I did a lot of research on this topic as i am trying to gather all information for my next step i-e AOS.

when you did your medical in Pakistan, they should have provided you a copy of form DS-3025 which only states all your vaccinations status.This form and other medical details was in sealed envelope which was handed over to immigration at airport by you. The immigration at airport forward this envelope to USCIS. So USCIS have that information that you have done your medical and vaccinations if you did in Paksitan. As you have done you medical and USCIS have that information, you don't need to do another medical for sure. Now the only thing is left is vaccination part.

Let say if you vaccination was competed, then you only need to submit copy of DS-3025 with your AOS. If you don't have DS-3025, then you will need to submit Form I-693. You can't filled I-693 of our own. A civi surgeon authorised by USCIS need to filled some parts n sealed in envelope. Now for civil surgeon to fill that vaccination part, he needs to see some sort of vaccination proof, which is of course ur Ds-3025 form, which you don't have it. You can call US Islamabad embassy and can request for your medical details and form DS-3025 which was handed over to them by your medical clinic where you took your medical exam. Few members have done that and Embassy have send all medical info in sealed envelope to them in USA.

Lets say if Embassy send you your medical file or DS-3025, then you should look what your form DS-3025 says. if you took at least on dose of all required vaccinations, then you don't need to go to civl surgeon and waste money.

Let say if embassy is not sending you then you only need to get necessary shots of IPV,Tdap, influenza and MMR. you just need to get one dose of each vaccine. you can get it from civil surgeon but their prices are high. You can ask public heath office in your area for these vaccines as their prices are very low. Take that vaccination proof to civil surgeon and he should charge you only for transcription.

Please note when you call Islamabad Embassy, tell them you took all your vaccines and your vaccination history was completed when you did your medical exam and you only need that DS-3025 form for AOS and you don't want to waste money again.

one more thing, you can also check with medical clinic in pakistan where you took your medical. Normally medical clinics have record for few months. They can you provide you DS-3025 copy and thats the only thing you need for you AOS.

Hey this was veryy helpful i check it out n then called USCIS customer service with the same question

and they said that since i dont have the vaccination supplement i would have to go and get the medical done

as she said that they need parts 1, 2 and 5 completed of the I-693 (the medical exam form) Therefore,

i have to make an appointment with a civil surgeon n get those filled out.....so i made an appointment

for an exam....but ughhhh this would be completely unecessary if i had the vaccination supplement from

Pakistan....anyways just hope everything goes well ...inshAllah

Thanks zee481 i just came across ur reply...n yeah i thought of that too but its a long shot cuz its Pakistan n they just take foreverrrrr for everything n like i asked my dad to contact the doctor who i got my medical done from n they said that since k1 is a nonimmigrant visa they dont give the vaccination supplement which makes no sense to me cuz what harm would it do if the just made one extra copy of that one page with vaccines, right? ughhhh so frustraing! so anyways ... after reading the earlier threads , courtesy of zabbas, i called USCIS n confirmed with them about what i had to do....n turns out...i gotta go down to Milwaukee from Green Bay n get a whole exam done which is gonna cost 225 dollars plus the cost n hassle of going there on the bus and then going back AGAIN after 2 days to pick up the results!!!! :/ i hope i can talk the doctor into just filling out my vaccination supplement n they dont have to draw blood etc... which is the part i completely dread!!

n oh yes i have my vaccination records from childhood n i'm up to date on my vaccines so i really dont wanna get 'em done again. thanks alot for ur help i greatly appreciate it.. :)

i don't know why USCIS said that you need to do whole medical when they have your record. USCIS website clearly states under " Medical examination ".

K1- If medical done within 1 year of applying for aos, then it should be good. You might have to get more shots to be up to date( my wife and children had too.) Do you have private insurance? If you do a lot of them pay 100% for Vaccinations, mine did. My understanding is the DS-3025 has to transferred onto to Form I-693 by a Civil Surgeon. Don't let them fool you by telling you you need a new physical and all this extra money. I called several and finally found one in the neighboring state for a grand total of $26 dollar per person. I did 3 visas.

When I say updated, meaning maybe more Vaccinations? It goes by what you already received and your age. Your physician could tell you if current but once again you need CS to fill out I-693. Didn't realize you were still in UK. Once you get here take my advice.

OMG i think those doctors were trying to rip me off by saying they don't fill out the I-693 without doing the full medical exam n they just wanna make money by charging $225 per medical exam.... cuz even the USCIS rep. told me i just need parts 1, 2 n 5 filled out. So it makes no sense in doing the whole medical exam if i got it done within the last year and it says excatly the same thing on the USCIS link you posted zee481....but ugh..i already made an apppointment with this doctor in Milwaukee, Wisconsin... n the lady from their office was like its gonna be a full medical n i have to come back 2 days later to get the tetanus exam checked n to pick up the results as they need two days for the blood work to come back n she said to bring $225 ..pffttt!!! ...so guys what do u say i should do ...should i call 'em back n ask why they cant do it seperately n for 1/3 of the price of the medical cuz in those threads those people got theirs filled out for under $30....OR should i just go n take a print out of the USCIS page where it states that about vaccination supplement filled out seperately, n show it to them if they dont budge??? this is really frustrating as me n my husband really dont wanna pay that much because we're in a tight spot financially.....

Arooj I too am having the same problem with my fiancee. The only thing is that she doesn`t have any of her vaccination records since her parents lost it a long time ago. The doctor did not issue her a vaccine since he said that it wasn`t necessary for a K1 visa. Sorry just needed to rent a little :-/ I will be following your thread
