is the VACCTINATION DOCUMENTATION needed for the AOS package?

Hello everobody!

It is takeing me a long time to work on the AOS package so I don't want to miss something and have to wait even longer after that. Is the VACCTINATION DOCUMENTATION needed for the AOS? I have a worksheet that says For Use With DS-2053 or DS-2054.

Needed? Technically no but as the I-693 is listed in the requirements it's best to include the DS-3025 to show the requirements were met with the K1

Thank you for your post! It is so helpful! But I'm still confused... Do I need another medical exam? I've been told that the one I had in Bulgaria is good for an year or so and just have to send the vaccination worksheet with the AOS package... :help:

No you don't need another medical if it's less than a year since your K1 medical. If your shots are complete you just send a copy of the DS-3025

The worksheet says "Vaccine History Incomplete" but above all I see for the vaccines I don't have is Not Age Appropriate. I thought that they would make me have the vaccines before the interview in the Consulate if there was something wrong. What should I do now?

You can get a K1 without vaccinations so it's very possible that you do not have the ones required for AOS.

Some people get the shots before their visa medical, not because they have to, but they want to take care of it before coming to the US. When they do that, their vaccination sheet from abroad is acceptable proof for AOS because it lists dates for all the required shots, it is filled out properly (like not age appropriate ticked on baby shots, VH marked if you had chickenpox, may be eligible for blanket waiver marked) and it was signed and dated.

If there is even one shot missing then you get it in the US and have a USCIS approved Civil Surgeon sign off on a new vaccination sheet--part of form I-693. You don't take a new TB test or get a full exam.

Thank you for the responses. I'm sure I have all the shots that I have to cause my mom is a nurse and won't let me miss a shot but may be there was something wrong with the written record I had. I will check and have the shots again if I have to. This 'Vaccine History Incomplete' will cause a RFE if I don't.

No it won't. It says "incomplete" because there are shots on the form that you don't have. Some shots aren't for your age so they're not needed. It is virtually impossible to get "complete". Complete is only if you have every single shot, which isn't required.

If YOU know your shots were complete the send it. You need tdap, MMR, Varicella. If you have those you should be fine.

"Vaccine history incomplete" is accepted, but ONLY if it also has checked "may be eligible for blanket waiver". Do you have both boxes checked?

Did you get any new shots since childhood? The Diptheria/Tetanus/Pertussis needs a date since 2001 beside it. Just because your mother kept up with your shots as a child doesn't mean you're okay for immigration as an adult.

Repeating from the other thread--

Look at your DS-3025. If it has the ALL of the following items, you don't need an I-693 (Adults 19-49 yrs old).

1.
Shots marked with a date (at least one of a series)

MMR

Td or Tdap or DT or DTP or DtaP
(
One no longer than 10 years ago
, ie.have a booster)

Varicella or VH
written by it if you had chickenpox

Influenza
**see note below because you are most likely okay without it.

2.
Everything else marked not age appropriate or insufficient time interval or contraindicated

3.
RESULTS section filled in with "incomplete" and "may be eligible for blanket waiver" ticked.

4.
Signed and dated by the doctor

If you can't look at your sheet and see if you have all those things (every detail) marked then you should go to a civil surgeon and ask him. He gets paid to read your sheet and help you figure it out. We can't see your sheet and words don't seem to be helping you.

It also says on the DS-3025 'Applicant may be eligible for blanket waiver because vaccination(s) not medically appropriate (as Indicated Above)'. The last shot on the Written record I had is dated in 2005 but I remeber have some after that. Unfortunately they are not on my record. Some doctors in my country don't keep up with the paerwork... I'm thinking about sending it the way it is. If there was something wrong the doctors in the clinic would tell me.
