ok, so here in California after I had called several Civil Surgeons to make an appointment (got turned down by some because they required a full medical too) I had finally made an appointment because someone actually knew what I was talking about, WRONG.

I printed out the 1-693 form, filled out my part (half of page 1) and took the rest to the appointment. The lady then told me I didn't have any proof for the other requirements such as the blood test and mental health #######. so I told her that I already had my actual Visa medical and all I required (which it says in the instructions) Page 1 and the vaccination page to be completed. SO then she said that she will complete only the vaccination page and not page 1. So then I just got frustrated and left, Has anyone else ran into this problem!? Also she wanted to charge me $125.

Any help guys would be GREAT!

Ugh, I saw these problems coming when they changed the form and instructions.

Do you have copies of your medical, other than the vaccination worksheet? Can you contact the medical clinic where you had your exam and see if they can send you a copy? It might be needed to prove to the CS that you had the exam and everything checked out fine for you. Even though you would not have gotten your visa and be here if something was wrong with your medical. The instructions still state you do not need a new medical, but page 1 of the new form is basically asking the CS to sign off that they know you had the exam and everything was good to go in it. It is a big circle of silliness for K-1 holders with this new form. Hope you can get it straightened out!

Were all your shots complete when you did the K1 medical? The form doesn't need "complete" ticked, you just need to know if you're missing anything and had to get it here. If you're not missing any shots then just send the DS-3025.

Unfortunately with the new form and the new instructions it makes CS's think that they need to see PROOF of your K1 medical. If possible call the panel physician back home and ask for them to send you a copy of your K1 medical. I went to Dr. Ograglik and he posted it to my family who posted it on to me.

Which part of California are you at?

There is a thread on the forum which lists all the civil surgeons that do not give you a hassle on it.

You should check it.

I paid $30 for mine in SF.

They only filled the vaccination sheet, and the first part with my details and the doctor's details.

They did not touch the "date of last medical examination" part because they said they did not do it.

They would not even write the date i did my last examination.

They let me check all the details, before putting it in a sealed envelope

i posted a topic about this yesterday too, although im all the way in florida i paid 60 dollars today but the CS read the instructions before she believed anything i was saying. I recieved an rfe for the medical and i dont recall ever getting anything "ds3025" so im hoping and if anyone can answer that for me i would be sooooo greatful! do i need to send that in with the rfe or does USCIS know i got my medical and jsut wanted proof of vacc. cause i know in ecuador the Civil surgeon didnt do that?!!?!

but my best advice is keep calling doctors and ask for prices but what worked for me is insteadof asking for the immigration physical i just asked for the vaccination transcript. good luck!

I don't have anymore copies of my medical, and I had my medical done in Sydney with Dr. A Millar who now is retired. Ugh.

My shots at the time of the medical were completed, I had to get booster shots after the medical though which i do have proof for. My DS-3025 is marked incomplete but is also ticket with "applicant may be eligble for a blanket waiver because vaccinations are not medically appropriate" And most of the vaccinations are ticket with "not age appropriate"

I'm in Los Angeles, Where is the forum with the hassle free civil surgeons? If you could link it please that would be fantastic :)

What boosters did you need? Sounds like that at the time of the medical you were as complete as you could be. I would send the DS-3025 and continue to research CS's. Then if you get RFE'd you can do it quickly. Many people send just the DS-3025 without issue.

Op - My wife got the form completed for $50 in SF. It took about 30 mins. We simply took the 3025 and CS just copied vaccination info. A full exam was not required since wife had physical back in June.
