Last name missing in Birth Certificate

I am an Indian-born Canadian Citizen married to a USC. After hunting around for my birth certificate, my parents told me that it has my last name missing. It has all what the NVC states from the travel.state.gov website:

Your date of birth

Your place of birth

Names of both parents

Indication by the appropriate authority that it is an extract from the official records

Unfortunately, it only has my first name and no last name. What to do?

My parents live in UAE so they aren't even sure getting it attested by the local embassy will help. Ideas??

Found this under the reciprocity agreement in the DOS website:

In cases where birth certificates from the authorities are unavailable or contain insufficient information regarding the birth or the parents, a sworn affidavit executed by either the parents, if living, or other close relatives older than the applicant, may be submitted. It should set forth the relationship between the deponent and the applicant, how well the deponent knows the applicant, date and place of the applicant's birth, the names of both parents, and any other related facts.
