hi everybody,

i found out that we need a civil surgeon to get our medical history or information onto a form i 693 which we are submitting with the AOS forms. please i am so confused about this whole thing right now because the only thing we have is the vaccination history and not any of the other medicals because our sealed brown envelop was taken at the POE.

can someone who has gone 2ru this plse help out as in what to do. tahnks

The vaccination history is all you SHOULD need for the transcription to the I-693, although a lot of civil surgeons will insist otherwise.

What does your DS-3025 vaccination sheet say - were the vaccinations marked "complete"? If it says complete then you do not need to see a civil surgeon at all, you just send a copy of the vaccination sheet in with your AOS.

I havent gone through this process and I dont know anyone who has by but how about going to a doctor here and asking for a physical so that you have some sort of medical record here.I vaguely remember doing a medical with a USCIS recognized doctor when I filed for permanent residency.Hope other people on this board will be able to give you more input.

No physical needed. Just need the vaccines transcribed onto the form. Ours cost about $120. He needed to get the flu vaccine (Arrived last december). If she had all of her shots done in Ghana she should be okay- but they will look it over.

Also for those who are from Ghana Tetonal is the same as Td. I had to do some research and the doctor confirmed- so if they say you need Td you dont.... save you a few bucks.

thank u so much. so do we just need to send the vaccination card to the surgeon for him to transfer it onto the i 693? and also can any doc do dat or it has to be done by only designated uscis civil surgeons?

You typical have to go (but i have heard some will let you send it and have it transferred without being there) but since you just got here and its flu season and Israel was required (last December)- i would find someone local so you dont send it in to then have to get more shot(s) and then have to re-send it...

and yes it needs to be one of the designated civil surgeons... if not USCIS will send it back...

Make sure its in a sealed envelope! We were also given a copy - make sure all information is correct on the copy- if not ask to have it re-done and re-sealed.

one more thing. do they need a copy of our marriage certificate or the original copy. and also will the petitioner and the joint sponser both fill the same i 864 or do it separately. and what about the w2. what is it and do we have to get it?

I sent original- some ppl have gotten RFEs for copies. Spend the few bucks for an extra (you are gonna need the originals for lots of things anyway. we got 4...)

Depending on how the joint sponsor is depends on what forms- but they would need two separate. There is an I-864a if they are in teh same household- read up...

Copy. I have NEVER seen ANYONE get an RFE for a copy. I've seen people say that they've seen people get RFE's but I'm yet to see a thread where it's happened.

Separate I-864's.

Contact the IRS for tax transcripts. It will save you a lot of confusion.

Could you please link threads where people have been RFE'd for a copy? The instructions state copy and I have never seen such a thread.

As for how many originals. We have one and have only ever needed one... this includes changing my name in my home country (drivers licence, bank accounts, passport) as well as changing my name here (SSA, banks, drivers licence, AOS).

I sent one copy of marriage certificate.

6 passport pictures. 2 for each application (read instructions from each form)- you need to submit it as three separate applications (can be in same envelope- but you need to provide them with each copy of things for each application. Read the I-485, I-131, I-765, and I-864 (I-864a if needed) USISC instructions. They are easy to follow and must be followed..... info on here is great- but do read them in the whole, because you will be informed personally so you can only blame yourself if you miss something...

my cover letter looked like this:

Contents include:

- Payment in the amount of $1070

- I-485

- Marriage Certificate

- G-325A

- Copy of Birth certificate of petitioner

- Copy of I-797 approval of Petition for K-1 Visa

- Copy of Passport Biography Page

- Copy of Visa and entry stamps from December 16th, 2010

- Copy of front and back of I-94

- 2 Passport pictures of applicant

- I-693 in sealed envelope

- G-1145

- I-864 and I-864a

- I-131 and supporting documents

- I-765 and supporting documents

Alot of the surgeons will giveyou a hard time. Most will absolutely not transcribe your record without you getting a exam from them. We got pretty lucky in MD. 2 off of our list agreed to do it. Whe you call around make sure to tell them you only need to have your vaccinations transcribe to the form. Alot times they do not know. Hopefully you find a surgeon who will. Far as copies. We sent a copy of everything and kept orignials. Usually when you have your interview you can show the originals. Never advisable to send your original documents. Now for the marraige certificate you can request more than 1 and sent an orignial if you like. We didn't We just had color copies made of everything. We didn't have interview. Thank goodness. Our case was transferred to CA for faster processing. It really was not any faster but we got to bypass interview and was approve for GC within 6 months. EAD and Advance parole was approve withn 3 months.

i other thing: Check your birth certificate. Was your birth register late. If so include some additional info so that you dont get RFE. Do you have school records with your birth? baptismal certificate with your birt? Medical records with your birth? That was the only thing that we got a RFE for.

I dont have time to search through the threads- but someone DID post an RFE, don't know the rest of the case but it had to do with a copy of the certificate...It might have been on another site too- there are other sites I looked at. I rather be safe then sorry. I am writing about my own experience, thats all- isn't that what this site is- we say what we did, it works, so we suggest it....If I took everyone's advice on this site I would not be in the great situation Im in. You have to do your own research and your own reading of official documents.... I have had no problems with anything in immigration due to being too PREPARED- so I rather go through this process and not deal with RFES (I got one for a missed check box way back). After the ####### in Ghana the many of us went through I rather be prepared and organized so that we can enjoy our lives together and not have to worry about things.... (Don't want to sound like a negative nancy here- just speaking from my personal experience of taking my own responsibility).

I got multiple copies and it may just be were i live, but you have to send it in (for some changes) and then get it back - I rather know I have one copy I can keep and if they screw up they screw up... Ghana can also be complicated to deal with, so thats why i also got multiples as well. At my city hall its cheaper if you buy a group of them then having to go back if its is destroyed...
