DS-156 DS-156K??????

Timeline

Does anyone know if the DS-156 and DS-156K forms are necessary for the Warsaw embassy??? According to there IV-F04 packet that they have on the embassy website, they only request I-864, I-134 and DS-230 part 1.

witam, packet IV-F04 to dopiero pierwsza przesylka z ambasady z informacja, ze dokumentacja do wizy K1 znajduje sie juz w Warszawie. Po odeslaniu DS-230 part1 i IV-04 przyjdzie przesylka informujaca o terminie wizyty w ambasadzie na interview i wtedy trzeba wziac na spotkanie dodatkowe wypelnione, niepodpisane form. przesylam link do strony ambasady US w Warszawie, gdzie znajduja sie wszystkie informacje nt. interview. W razie czego sluze pomoca. Zycze powodzenia.

We did both, 156 and 156K...Go figure why they need both. We had interview in July. Also notice they wanted the 130 affidavit notarized (this is specific to Warszawa apparently); though other couple from Ukraine that had interview also in Warszawa noted that it was not required notarized for them. We didn't want any risks/details.

You don't really need to wait for packet 3 or 4; since all that is posted in web site of Embassy, you can send ahead of them sending you the forms. The site also has a list of the doctors for the exam and other requirements. You can schedule your appointment and have things ready for interview.

They schedule interviews for the entire month at once during the last week of prior month.

Finally, you can actually call them -free call- at +(48)(22)625 10 42 Monday through Friday, between 10:00 am and 12:00 noon and between 2:00 pm and 4:00 pm. Except holidays. You will get an actual person and can get direct answers.

Thanks but we also need the I-130 form for K-1 visa??? I found the checklist and documentation needed for fiancee visa and can't find were it say's we need the I-130. :(

My apologies, it is I-134 not I-130... :bonk:

Thanks again. I just found the IV-F05 package on the embassy website. The only thing that I find interesting is that in english it doesn't mention anything about the I-134 form being notorized.

"dla osób ubiegających się o wizę narzeczeńską – podpisane i potwierdzone notarialnie gwarancje na

formularzu I-134 wraz z zaświadczeniem z banku i o zatrudnieniu (opcjonalnie, ale rekomendowane) ."

I think the embassy should look into this.

I think the embassy should look into this.

Exactly, though I recall seeing the 'notarized' as well elsewhere in the web site.

Anyhow, we wanted no excuses/risks, so I went to my bank and notarize it, I had to sent an original signature anyway. Interview is quite simple (or was in our case); took 10 minutes. Good luck to you guys!
