Hello all:

I'm a little confused about the documents that should be included with our AOS package. Specifically the 1-693 and what the AOS Insturctions term the Vaccination Supplement. I'm not sure if this is a specific form or something we should have received at our K-1 medical exam. Can someone clarify?

Also, are the other forms required the _485, I-864, I-765, and I-131? Am I required to complete the I-130?

Someone help please. I have reviewed the guides, but I'm still unsure.

Thanks.

I-130 has nothing to do with AOS from a K-1 the step by step guide is pretty clear.

The vaccination sheet is something you should have gotten a copy of at the medical basically a sheet of boxes with your vaccinations marked off and signed by the doctor. If your vaccinations were not completed you would take your vaccination sheet with the 693 vaccination section to the civil surgeon.

Your I-129f was approved in 5 days from your NOA1 date.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.

The last page of this pdf is the DS3025. He should have gotten a copy of it, but many people have not gotten this back in JA.

Do you have his vaccination card showing what vaccines he was given?

You will have to take it to a civil surgeon and they will complete the I-693 form:

I will try to find the thread where one of our members had to go through this in the morning... mi tired now. Hope these links help in the meantime.

We sent my wife's AOS with only the DS-3025 which said "Incomplete, may be eligible for a blanket waiver" and there was no problem (no RFE) and AOS was completed without an I-693 (other than the original K-1 medical in her home country).

If the medical took place less than one year before filing for AOS, it's usually not absolutely necessary to have them transcribed. I have heard that some do receive RFEs, but it's up to you to decide whether you want to pay a CS to transcribe them upfront or to skip the CS and, if for some reason you do receive an RFE, if a 2-3 week delay to visit a CS is worth it...

We decided to skip the CS and if worked out fine...
