Thank you. My wife is USC, and we file tax jointly - do we both have to fill out the form 134 each or we can put both our names on the form? Who fills out what?

I-134 is for an individual only. No combining of incomes with the I-134. Use only your income information.

Hi understanding that it has to be individual, what about the tax document? I filed jointly with my wife because we are married. Can I use that tax and fill out the form I-134 with only my information then?

You provide your joint taxes and your individual W-2 that shows your income alone.
