When filling out the forms.....

When my fiancee fills out the forms for adjustment of status, does she need to use her maiden name on all forms, like G-325A, I-765 and I-131 or just on the green card application which is I-485.

She needs green card on her new name.

01/20/2012 - Hard copy of transfer received

01/23/2012 - Biometrics done

02/21/2012 - EAD and AP approved

02/28/2012 - EAD and AP received on same card

03/15/2012 - Green card approved, in production

03/20/2012 - Green card in hands

She will fill out everything with her new married name, and whenever a form asks for a previous names or maiden name, she will give her maiden name.

12-07-2011 E-Notifications via Email and Text - EAD and AP approved (in 64 days)
