When my fiancee fills out the forms for adjustment of status, does she need to use her maiden name on all forms, like G-325A, I-765 and I-131 or just on the green card application which is I-485.

She needs green card on her new name.

12/22/2011 - 3 emails and text notifications

12/23/2011 - Check cashed

12/27/2011 - NOA1 hard copies received

12/30/2011 - Biometrics letter received, appt date set for Jan 23rd

01/17/2012 - Case transferred to CSC (notified by text and email)

01/20/2012 - Hard copy of transfer received

01/23/2012 - Biometrics done

02/21/2012 - EAD and AP approved

02/28/2012 - EAD and AP received on same card

03/15/2012 - Green card approved, in production

03/20/2012 - Green card in hands

She will fill out everything with her new married name, and whenever a form asks for a previous names or maiden name, she will give her maiden name.

01-24-2011 NOA1 hard copy received in the mail [dated 01-18-2011]
05-12-2011 NOA2 (approved in 114 days)
05-18-2011 NVC received, new case number assigned
05-20-2011 NVC left
06-01-2011 Embassy received
06-06-2011 Packet 3 sent
06-10-2011 Packet 4 received
07-21-2011 Interview at 9:30am - APPROVED!!
07-29-2011 Visa in hand
10-02-2011 Wedding
