Civil Surgeon Overview for K-1 Holder

I spent the last two days looking for information for civil surgeon to transcribe my vaccination record without medical check up and I couldn't find any. I went through all the USCIS designated civil surgeon and make inquiries. Some insisted to get a full check up and the others requested skin (TB test) and blood work done. Here is the summary and I hope it can help others around the areas.

I spoke to Yvonne that informed me $72.50 for blood & skin test and paperwork. There will be extra fees for immunization (MMR and Tetanus) in total $216.50. The office hours Monday to Friday 8.30 am to 5pm, Saturday 7.30 am to 1pm (No need to make an appointment). They accept only cash, debit and credit card.

I spoke to Bertha ( very patient and helpful) that informed me $95 for blood & skin tests and paperwork. There will be extra fees for immunization; MMR cost $60, Tetanus cost $30 and Varicella $110. Discount 10% will be $285.

I spoke with Yolanda (very helpful) that informed me $110 (in cash) for blood & skin tests and paperwork. I need to go another clinic across the street for immunization. Please call to make appointment since the latest immigration patient they can take is at 2 pm. The procedure come in for registration, have blood and skin test, wait 2 days, come back and meet the CS for necessary vaccination, get them across the street and come back for signing.

I spoke with a nice lady called Lupe that informed me $165 for blood and skin test, paperwork. There will be extra fees for immunization; MMR cost $82, Tetanus cost $53 and Varicella $140. Operation hours Monday to Saturday: 7.30 to 9pm. They open on Sunday too.

We never went to a civil surgeon in the US. We only sent the vaccination worksheet that we got from the medical in Canada. No problems. Did you get something similar in Indonesia?

Thanks for quick reply. I read since I only have the vaccination worksheet without a copy of my DS3025 form from my clinic, I will need to see a civil surgeon for them to transcribe the record to I-693 form. Did you send a copy of DS3025 along with the vaccination record from your clinic?

From the K-1 guide: "Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination."

If they insist that you have a medical and you had your K-1 medical exam less than a year ago then they're wrong. They should just transcribe your vaccinations if that's all you need (If you hadn't already got all your vaccinations at your K-1 medical exam). As far as I know you can get the required vaccinations anywhere you like, and just bring proof to the civil surgeon.

I'm just talking from my own experience. I don't know if there's new rules requiring a TB test and blood work. They should have been done already when you had your K-1 medical.

You should only need a copy of the vaccination work sheet from your previous medical in your country. (All I sent was the vaccination sheet, no rfe or anything)

You only need to see the civil surgeon if your vaccinations are incomplete. Or if the POE lost your entire file of your medical. Otherwise no need to see the civil surgeon.

Thank Inky for your quick reply. Let me clarify, you sent the immunization card without the DS3025 form? My immunization card was left blank for Varicella, MMR and Tetanus. So, my vaccinations are incomplete. I am not sure I can send it without seeing the civil surgeon.

Ah yea then since your vaccination is not complete you will need to get the civil surgeon sadly =(

I just got back from Dr. Ramirez office. Total turn around time around 1 hr. I had my Mantoux test first, then MMR vaccination followed by blood test and the physical test/interview the latest. I will need to come back on Friday. Overall it was quick, the office was neat, the reception was nice, the nurse was patient and the doctor had a good sense of humor.
