Question 38 reads (in part): "Have you ever been refused admission to the US, or been the subject of a deportation hearing ...."

With respect to the first part, being refused admission, is being refused a visa the same thing? My fiance was denied a tourist visa in the past at the US embassy in her country, which could be thought of as being refused admission, but I suspect they are referring to being refused admission at a port of entry.

Being refused a tourist visa is NOT the same in regards to the DS-156 question of having been denied admission.

You are correct that being denied admission occurs at the POE.

So, the correct answer for the DS-156 is "no".
