OPT and filing I-765

I applied for post-completion OPT and mailed my application and supporting materials to USCIS Dallas lockbox. It was delivered on Nov 20th. Unfortunately, I think I made a mistake on form I-765. I had previously applied for OPT after my Masters program, and received an EAD that authorized me to work from 9/2/04-9/1/05. I then stopped OPT and began the PhD program at Morehouse School of Medicine in July 2005 and I am now applying for the OPT after completion of my PhD. I guess you are allowed to apply for OPT for each degree level. Based on this I selected "Renewal of my Permission to accept employment" Instead of "Permission to accept employment" under the I am applying for box on form I-765. Do you think this will be a problem and how can I correct it? I have already mailed out the form. On reading the I-765 instructions, renewal was defined as an "EAD issued to an eligible applicant upon expiration of a previous EAD issued under the same category" so in my case the category is Post completion OPT and because I have had an EAD at a previous lower degree level which was also post-completion OPT, I assumed I was applying for a renewal of my permission to work. Please advise me if you can on what I can do in case I have selected the wrong option. Reading blogs and instructions from other schools is convincing me that I may have selected the wrong option.

Also, I made a mistake regarding my place of entry on Item #13. I wrote Atlanta GA and on inspecting the stamp in my passport it says NYC. Could this present a problem?

Please advise me if you can as I am getting no reliable advice. I am worried about these 2 mistakes and whether they will result in delay or denial of my application. Can I contact USCIS and correct the issues?
