Montreal ?'s ---DHL, Expedited Interview for CR1

So....we are in the final stages of the CR-1 visa approval for my canadian husband. He finally received an interview date for December 19th in Montreal. Here are my questions for anyone that may have the answers.

1. We are needing to appeal for an expedited medical emergency interview date sooner then the 19th of December. We've been told to email everything to montreal-niv@state.gov OR montreal-iv-dv@state.gov with all the proper documentation, etc. Has anyone ever inquired thru Montreal recently for an expedited interview date and who did they send it to and how did it turn out??

2. Secondly we see all of these messages regarding a pre-registration with DHL. We've done everything electronically thru the NVC in the USA so they setup our appointment with Montreal and we submitted all electronic documents, etc which means we received our interview date via email .pdf as well. Nowhere and I mean nowhere in any .pdf we've received from the NVC does it have information to signup for DHL. Is this only required for certain visas??? And has anyone that used DHL to Vancouver tell me how long it took to receive their packet & passport back?

In P4 (Interview Notice), we were instructed to go to http://www.immigrantvisa.state.gov to find out more about the process. Select "Prepare for your visa interview abroad." From there, you can select Embassy/Consulate-Specific Interview Guidelines for Montreal. That brings up this PDF:

From that document, I read:

After the interview

If your visa application is approved, we will put your visa in your passport and send the completed visa packet and passport to you by DHL courier at no extra charge. *Return of your passport with visa and the immigration documents package: If your visa application is approved, we will put your visa in your passport and send the completed visa packet and passport to you by DHL courier at no extra charge. Prior to your appointment date, you must go to http://canada.usvisa-info.com/ and follow the steps to register at which DHL location you would like to receive your visa. Please do this for all applicants that will be applying for a visa (including children). Please register your delivery location BEFORE going to your interview. If you do not, there may be a delay in the return of your visa.

Jul 11, 2011: I-130 approved
Aug 18, 2011: NVC process begins
Oct 6, 2011: NVC process completed
Nov 4, 2011: Interview date assigned
Dec 2, 2011: Interview at Consulate (Result: AP - No reason given on 221g)
Feb 7, 2012: Passport requested by Consulate
(...Husband needed passport for personal reasons during this time...)
Apr 9, 2012: Passport mailed to Consulate
Apr 25, 2012: Visa issued
May 1, 2012: Visa in hand
May 31, 2012: POE at Montreal's Trudeau Airport
Early July: Physical Green Card received in mail. Yes, it's green!

March 13, 2014: ROC filed (went to VSC)

March 17, 2014: NOA

April 22, 2014: Biometrics appointment

December 9, 2014: Case inquiry initiated

December 11, 2014: Response: "The processing of your case has been delayed. A check of our records establishes that your case is not yet ready for decision, as the required security checks remain pending."

March 14, 2014: Received NOA that approval had been given on March 10; expect new GC within 60 days.
